No Referral Needed to Get Started
At Summit Physical Therapy, care is built around you. One-on-one sessions, thoughtful treatment plans, and a team focused on helping you move better and stay active long-term.
If Your Insurance Requires a Referral
Some insurance plans may still require a referral for coverage. If that’s the case, your provider can complete a referral form and send it to our office.
The form includes basic patient information, diagnosis, and treatment recommendations, and can be faxed directly to our team for processing.
Direct Access
In Florida, you can start physical therapy without a doctor's referral. As a Direct Access state, Florida allows patients to be evaluated and treated by a licensed physical therapist for up to 30 days before a referral is needed for ongoing care.
